To Add a New Line of Service:

To Order by Phone: Call 877-845-5317. Identify yourself as a Life Time Fitness member and provide promo code 9972TMOFAV. Please be ready to provide your LTF membership number. 

To Order Online: Click on the Redeem Online button above and follow the instructions to set up new service.

To Add the 15% Discount to your Existing Account:

STEP 1:  Click here  to access the T-Mobile online order portal. At the bottom of the page click on the box that says "Already a T-Mobile customer?" The online Corporate Discount Form loads.

STEP 2: Complete all fields and be sure to enter Life Time Fitness in the Parent Organization field. Click submit at the bottom of the form.

- Your 2 year service contract will be reset to two years from the date you apply for the discount. This does not affect your eligibility date for a handset upgrade.

- Your bill cycle close date will change to the 26th of each month. Discount will appear in the 'Adjustments' section of your bill within 1-2 billing cycles. The first discounted invoice will cover a partial months usage.

- All membership information will be verified through Life Time prior to being submitted for processing.

Discount does not apply to T-Mobile customers on Flex Pay and Pre-Paid service or customers on Unlimited Loyalty plans, Even More Plus Plans and Grandfathered rate plans. Members on a non-qualifying plan may change to a qualifying plan in order to receive discount.

Handset upgrades are not discounted under the member discount program.
